We have listed below a list of insurance companies in which Gwinnett Pediatrics maintains a billing relationship. While we attempt to keep this list current, it is subject to change without notice and is provided for general informational purposes only. The ability to bill a given insurance company for services will be assessed at the time of the office visit. Although a company is included on this list, it does not guarantee that your insurance company will pay for some/all of your services, nor does it obligate GPAM to accept deferred billing for any of the insurers listed. A valid insurance card is required to be presented at the time of each visit, along with the appropriate payment. After the time of service, GPAM will send a claim form to your insurance carrier. GPAM files a claim to your insurance carrier as a courtesy. It is up to the patient to know their own insurance coverage. Ultimately, it is the patient's responsibility for any balances owed to Gwinnett Pediatrics and Adolescent Medicine. Your copay and/or deductible is due at the time of service. A billing fee of $25.00 may be charged if not paid on the date of the visit.
If your insurance company is not listed, we may not have an agreement with them. In such cases, the account will be entered as a "self-pay", with payment due from the patient and expected at the time of service.

We take Alliant only if processed through PHCS - if there is not a PCHS logo on your card, we do not take it. We are also NOT in network with Ambetter, Wellcare, or Assurant.
We are currently not accepting any NEW patients on state health plans such as Peachstate or Peachcare.
