New Office Policy for Completing Forms

 Dear Families,

As a benefit to our patients, Gwinnett Pediatrics and Adolescent Medicine will complete sports, camp, school and after-school forms as a free service during a patient’s well-child office visit.  However, due to the time and expense incurred in reviewing patient files and filling out such forms, as of January 31, 2009, a $10 fee will be charged for all such forms submitted for completion outside of a well child office visit.  If a child has not had a recent well child check up a visit may be required prior to completion of sports physicals.

Although, we have previously made exception and filled out such forms free of charge for our patients outside of an office visit, we are no longer able to dedicate the time necessary to properly fill out such forms free of charge.
